Evaluations

An evaluation is a formal review of an employee's overall performance at work.

The Work Based Learning Student Agreement is used by a case manager to establish goals for an employee after visiting the trial work site and talking with the supervisor.

The Work Based Learning Employer Evaluation is given to a supervisor at a trial work experience in order to receive feedback about how someone is doing at work.




1) Upon completing a trial work experience, obtain the Work Based Learning Student Agreement and Employer Evaluation from your case manager.

2) File both documents in your Portfolio with the other Work Based Learning information for your work site.
